  • Notary Public  (for health care related documents)

Contact the Patient Representative at (865) 331-1611, Monday-Friday, 7:30 a.m. – 4:00 p.m.  After hours calls can be made to Nursing Supervisor by dialing “0” if you are inside the hospital or by calling (865) 331-1111.
