Self-management education program
Our team of professionals includes dietitians, nurses, exercise physiologists and a counselor. They create an individual self-management plan for each patient.
Whether you have Type 1, Type 2 or gestational diabetes, we will help you with:
- Nutrition
- Physical activity
- Medication
- Blood sugar monitoring routines
Part of your self-management plan is learning how to lower the risk of diabetes-related complications. We help you develop healthy strategies for this. The Fort Sanders Diabetes Center also offers monthly support groups for patients and family members. Our program has received recognition from the American Diabetes Association for Quality Self-Management Education.
Referrals
Most health insurance plans, including Medicare, provide coverage for diabetes education. Our staff at the Fort Sanders Diabetes Center will gladly help you with the referral process.
